After you have completed the assessment Phio passes all your information back to your healthcare provider. On the final page Phio will have given you an expected timeframe for when you should hear back and what the next steps are. If you haven't heard from your provider within the expected timeframe please get in touch with them. If you are not sure who your provider is, please fill in the below form and we can give you the right contact details.
